Infrastructure Improvement Projects Supported by the Nonprofit Infrastructure Capital Investment Program (NICIP)

Chautauqua Hospice and Palliative Care is proud to announce the opportunity for qualified contractors to participate in several infrastructure improvement projects, made possible through funding from the Nonprofit Infrastructure Capital Investment Program (NICIP). These projects aim to improve the functionality, safety, and sustainability of our facilities, ensuring that we can continue to provide exceptional care to our patients and their families.

The projects outlined below have been divided into four categories, and each has its own Request for Proposals (RFP). Contractors are encouraged to review the RFPs carefully and submit proposals based on their expertise and qualifications.

All proposals must be submitted according to the guidelines outlined in each RFP. Proposals will be evaluated based on cost, experience, proposed timelines, and alignment with project goals.

HVAC Replacement RFP

Project Title: HVAC Replacement
Location: 20 W Fairmount Ave, Lakewood, NY 14750

Introduction:
CHPC seeks a qualified contractor to replace the aging HVAC system at its main facility, ensuring improved comfort and energy efficiency.


Scope of Work:

  1. Remove and properly dispose of the existing HVAC units.
  2. Install energy-efficient furnaces, air conditioners, and ventilation systems.
  3. Test , balance the system, and verify system performance upon completion.
  4. Ensure compliance with all local and state safety standards.

Proposal Requirements:

  1. Company Information:

    • Legal name, address, and contact information.
    • Licensure, certifications, and insurance.
  2. Detailed Project Plan:

    • Equipment specifications, timeline, and key milestones.
  3. Cost Estimate:

    • Itemized breakdown of costs.
  4. References:

    • Three projects of similar scope, with client contact information.
  5. Minority- and Women-Owned Business Enterprises (MWBE) Status/Involvement:

    •  Certified MWBE or documented plan to include MWBE subcontractors

Evaluation Criteria:

  1. Proposed Cost (40%)
  2. Experience and Qualifications (30%)
  3. Work Plan and Timeline (20%)
  4. References (5%)
  5. MWBE Participation (5%)

Roof Shingle Replacement RFP

Project Title: Roof Shingle Replacement

Location: 20 W Fairmount Ave, Lakewood, NY 14750

Introduction:
Chautauqua Hospice and Palliative Care (CHPC) seeks proposals from qualified contractors for the replacement of roof shingles at our main facility. This project is essential to maintain the integrity of the building, prevent leaks, and improve energy efficiency.


Scope of Work:

  1. Removal of Existing Shingles:
    • Remove and properly dispose of all existing shingles, nails, and roofing debris.
  2. Inspection and Repairs:
    • Inspect roof decking for damage and replace compromised sections as necessary.
  3. Underlayment Installation:
    • Install high-quality underlayment to protect the roof decking.
  4. Shingle Replacement:
    • Install high-quality, weather-resistant shingles (e.g., architectural asphalt shingles).
    • Include ridge vents, drip edges, and flashing to ensure a watertight seal.
  5. Final Cleanup:
    • Ensure all roofing debris is removed from the property and surrounding areas.
  6. Minority- and Women-Owned Business Enterprises (MWBE) Status/Involvement:

    •  Certified MWBE or documented plan to include MWBE subcontractors

Driveway Paving, Back Patio Replacement, and Sidewalk Repair RFP

Project Title: Driveway Paving, Back Patio Replacement, and Sidewalk Repair
Location: 20 W Fairmount Ave, Lakewood, NY 14750

Introduction:
CHPC seeks a contractor to complete several exterior projects, including parking lot paving, sidewalk repair, back patio replacement, and the construction of a connecting road to the Star Hospice House.


Scope of Work:

  1. Main Parking Lot Paving:

    • Milling Required
  2. Sidewalk Repair:

    • Replace approximately four (4) sidewalk blocks.
  3. Back Patio Replacement:

    • Replace deteriorating concrete back patio and steps
  4. Connecting Road:

    • Excavate and pave approximately 15’ x 100’ road from the main parking lot to the Hospice House property line.

Proposal Requirements:

Proposals must include:

  1. Company Information:

  2. Detailed Work Plan:

    • Outline of the process for each part of the project, including materials and equipment.
  3. Timeline:

    • Proposed start and completion dates for each section.

    • Three similar projects completed within the last three years.
  6. Minority- and Women-Owned Business Enterprises (MWBE) Status/Involvement:

    •  Certified MWBE or documented plan to include MWBE subcontractors

Project Title: Solar Installation

CHPC seeks proposals for the design and installation of a solar energy system to enhance sustainability and reduce energy costs.


Scope of Work:

  1. Conduct a site assessment to determine optimal solar panel placement.
  2. Design and install a solar array for the roof and surrounding parking lot.
  3. Integrate the solar system with existing electrical infrastructure.
  4. Obtain all required permits and ensure compliance with local and state regulations.
  5. Provide training for staff on system operation and maintenance.
