Full Time On-Call Registered Nurse:
Position Summary
Responsible for working with RN Case Managers to ensure that care is available and provided on a 24hr/7day per week basis, which addresses the physical, emotional, psychosocial, and spiritual needs of patients and families who are experiencing life-limiting, progressive illnesses.
(3) 12 hour shifts overnight
Essential Duties and Responsibilities
a) Utilize the nursing process to assure a comprehensive plan of care;
b) Assure ongoing assessment and revisions to a patient’s plan of care;
c) Teach patient, family and/or staff on caring for patient;
d) Assess patient, family and/or staff comprehension and ability to provide appropriate care;
e) Identify significant clinical findings, draw conclusions and intervene appropriately;
f) Promote communication through all levels of patient care including completing call log and on-call report;
g) Participate in and attend clinical meetings;
h) Give and receive “report” (status changes and needs of patients and/or families) to
appropriate Case Managers at the beginning and end of each shift;
i) Contact answering service at the beginning of each shift to confirm contact information, telephone number(s) where one can be reached;
j) Contact administrative backup nurse appropriately for direction on administrative questions;
k) Conduct or refer to administrative backup nurse admission/ informational contacts with patients and their families;
l) Support and maintain regulatory compliance;
m) Maintain timely and appropriate records/documents per Hospice policies and procedures;
n) Maintain/enhance professional skills and requirements including education and training;
o) Perform other tasks and duties as assigned;
p) Take initiative as needed or when the opportunity arises;
q) Provide on-call services and patient visits, as scheduled;
r) Travel as required.
Formal Education Requirements
At least an Associate’s Degree in Nursing is required.
Formal Experience Requirements
At least 1-2 years’ Medical-Surgical experience as a Registered Nurse is preferred

Demonstrates the use of effective verbal, non-verbal and written communication with patients, families, members of the healthcare team and community in order to therapeutically address and accurately convey the hospice/palliative care needs of patients and families.
Additional Skills/Abilities/Requirements
Working Conditions and Environmental Hazards
This position works in contact with risks associated with blood-borne pathogens and needle sticks. Care must be taken to follow all health and safety requirements.
Physical Requirements
Position requires a moderate-to-high level of independent judgment in performing responsibilities. Position also involves moving and lifting patients, and carrying light loads and supplies with or without assistance.
