Each holiday season Chautauqua Hospice & Palliative Care presents our Light A Life Memorial & Service of Remembrance as a tribute to loved ones who have passed. This is a time of remembrance not just for the families we have served; we invite all community members to participate regardless of whether or not we cared for their loved one.
There are several changes from previous years for the ceremony and ornament purchasing. Please read this information very carefully. The important changes include the LAL Service being streamed live online only (no in-person attendance) and ornament orders will only be available online.
On Saturday December 5th at 6pm we will held our Light A Life Memorial Service and Tree Lighting Ceremony at Chautauqua Hospice and Palliative Care. This year’s service was only streamed online, in consideration of Covid-19 restrictions. The video link and program are now available to watch here. Our Memorial Tree and Light Towers are now illuminated during the evening.
Those being memorialized will be listed in the Service of Remembrance Program and Light Towers. Individuals who have passed under our care in the last year will be listed automatically. Community members are invited to memorialize their loved ones who were not cared for by CHPC in one of two ways: by purchasing a holiday ornament in their memory or by submitting their name to us in the online form below.
